Referrals

CRITERIA FOR YOUTH REFERRALS

 Since 1992 over 300 youth have been referred to the One-to-One Mentoring Program by our schools and community agencies.  Partners does not solicit youth—we depend on trained professionals from local youth services agencies to recognize the at-risk youth who could benefit from a one-to-one relationship with an adult mentor.   

Below is a list of criteria for youth to be eligible to participate in Montezuma County Partners One-to-One Mentoring Program: 

  1. Between the ages of 8 and 17
  2. Been seen or identified by an agency professional as at risk of becoming or currently involved in the juvenile justice system
  3. Otherwise identified as an at-risk youth due to:

*youth has exhibited delinquent behavior

*youth is a drug and/or alcohol user

*youth is/has been pregnant or fathered a child

*youth demonstrates anti-social behavior

*youth demonstrates risk taking sexual behavior

*youth is a victim of neglect or physical, sexual or emotional abuse

*youth has experienced repeated difficulties in school

*family has significant drug or alcohol issues

*there is/has been violence in the home

*a family member is/has been incarcerated

*the family is economically disadvantaged

*the child is at risk due to some other identified factor and would benefit from services

 

All referred youth must enter into the One-to-One Mentoring Program voluntarily.

 Parents/Guardians must sign a release form for the referral to be considered. 

Once a referral is received, the Case Manager will arrange for an interview with the child, after which it will be determined if the youth is appropriate for the program.  If so, the youth will be placed on the waiting list, and is welcome to attend any of the planned activities with Partners.  Once we find an appropriate adult volunteer, the youth will be matched in a year long, one-to-one relationship with the Mentor.
